Aspiring Youth Academy Release and Waiver of Liability
This Release and Waiver of Liability (“Release”) applies to participation in programs, events, and activities organized by Aspiring Youth Academy (AYA), a 501(c)(3) nonprofit corporation in Arizona. By agreeing to this Release, you acknowledge and accept the following:
1. Waiver and Release
I hereby release and forever discharge AYA, its officers, directors, employees, volunteers, agents, successors, and assigns from any and all liability, claims, costs, or demands of any kind, known or unknown, that may arise out of or relate in any way to my child’s attendance or participation in AYA programs and activities. This includes, but is not limited to, any claims for bodily injury, personal injury, illness, death, property damage, or loss—whether caused by negligence of AYA or otherwise. I understand that by agreeing to this Release, I am giving up the right to bring legal action against AYA related to these matters.
2. Photographic and Media Release
I understand that my child may be photographed, videotaped, or otherwise recorded during AYA events. I grant to AYA all rights, title, and interest in these images, videos, audio recordings, or materials created by my child while participating in AYA programs. I consent to the use of my child’s name, likeness, image, voice, or work by AYA in perpetuity, in any medium (print, digital, online, or otherwise), for promotional, educational, and marketing purposes without further notice, approval, or compensation.
3. Miscellaneous
This Release represents the complete understanding between AYA and me and supersedes any prior agreements or understandings. If any provision of this Release is found to be invalid, the remaining provisions shall remain in full effect. This Release shall be binding on me, my heirs, executors, legal representatives, successors, and assigns.
4. Governing Law
This Release shall be governed by and interpreted under the laws of the State of Arizona.
By checking the box during registration (or signing below, if on paper), I acknowledge that I have read, understood, and voluntarily agree to the terms of this Release. I understand that I am waiving substantial legal rights, including the right to bring claims against AYA. If the participant is under 18, I affirm that I am the parent or legal guardian and that I have authority to provide consent on their behalf.
